A parent's signature is REQUIRED for any rider that is 17yrs or younger to participate. 
If a parent of legal guardian WILL NOT BE WITH the rider at the track, please follow the instructions below. 

WAIVERS 

GRASSROOTS MX WAIVERS -  (RMX Series is a division of Grassroots MX, LLC)​

Forms must be notarized to be accepted. 

Both parents signatures are required for the Annual Release

(unless you have sole custody which must be notated and notarized.) 

If a rider is under 17 years of age or younger  and parent or legal guardian CANNOT be with the rider at the event, he/she must have the Minor Release and Waiver of Liability Parental Consent Form signed by the parent or legal guardian 
THIS FORM MUST BE NOTARIZED TO BE ACCEPTED.  A minor child will not be able to participate in any way without this notarized waiver.  There will be no exceptions.
